Complete the form and add any notes about your purchase that we should know. When complete, click PROCEED TO PAYPAL. (We use PayPal to help us process your online credit card payment. You do not need a PayPal account) On this new page, you can decide to enter your credit card information and pay. Or if you do have a PayPal account and want to use it, choose pay with PayPal.

2. Want to mail in your payment? – – Download form here. Print, complete, and mail the form along with your payment.
